O objetivo deste estudo foi verificar a eficácia da Terapia Fotodinâmica (PDT) no tratamento de bolsas residuais de pacientes portadores de periodontite crônica em manutenção periodontal, através da avaliação de parâmetros clínicos e microbiológicos. Um ensaio clínico aleatório controlado duplo-cego paralelo foi conduzido para avaliar 34 pacientes com periodontite crônica, que foram submetidos ao tratamento periodontal convencional e apresentaram pelo menos quatro sítios com bolsas residuais na reavaliação. Os pacientes foram alocados em grupo teste (PDT) e grupo controle (sham) aleatoriamente. A terapia fotodinâmica foi realizada através da irrigação subgengival com azul de metileno (0,01%), e a irradiação com laser de diodo a 660nm, 90J/cm2 por 90s A intervenção foi feita no início do estudo, aos 3, 6 e 9 meses. Os parâmetros clínicos foram avaliados no início, 3, 6 e 12 meses após o tratamento. A coleta de placa subgengival foi feita antes da intervenção, uma semana após, 3, 6 e 12 meses depois. A avaliação microbiológica foi feita a partir da detecção e quantificação de Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola e Aggregatibacter actinomycetemcomitans, pela reação de polimerase em cadeia em tempo real. Todos os parâmetros clínicos avaliados apresentaram melhora significativa durante o estudo, porém não houve diferença significativa entre os grupos. Em relação à análise microbiológica, não houve diferença entre os grupos em nenhum momento do estudo. Porém, houve redução significativa de T. forsythia no grupo teste aos 6 meses, e aumento significativo de P. gingivalis no mesmo grupo aos 12 meses. Dentro dos parâmetros do laser e fotossensibilizador utilizados neste estudo, a PDT não demonstrou ter efeito benéfico adicional no tratamento de bolsas periodontais residuais.

The aim of this study was to investigate the effectiveness of Photodynamic Therapy (PDT) in the treatment of residual pockets of chronic periodontitis patients in periodontal maintenance, by assessing clinical and microbiological parameters. A randomized clinical controlled double-blind parallel trial was performed to assess 34 periodontal patients, who received conventional periodontal treatment and presented residual pockets at revaluation. The patients were allocated to test (PDT) and control (sham) group randomly. The photodynamic therapy was performed by subgingival irrigation with methylene blue (0.01%), and diode laser irradiation (660nm, 90j/cm2) during 90s. The intervention was performed at baseline, 3, 6 and 9 months. The clinical parameters were evaluated at baseline, 3, 6 and 12 months. The subgingival plaque sampling was collected before intervention, a week after, 3, 6 and 12 months later. The detection and quantification of Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola and Aggregatibacter actinomycetemcomitans was achieved by means of the real time polymerase chain reaction. All clinical parameters showed significant improvement during the study, but there was no significant difference between groups. The microbiological analyses showed no differences between groups in any time of study. There was a significant reduction of T. forsythia in test group at 6 months, and a significant increase of P. gingivalis in test group at 12 months. Considering the laser and photosensitizer used in this trial, PDT failed to demonstrate additional benefits in residual pockets treatment.
