O objetivo deste ensaio clínico cego randomizado foi de observar a influência das terapias com eletroacupuntura e laser-acupuntura no retorno da sensibilidade tátil e dolorosa após cirurgia ortognática combinada e mentoplastia. Foram selecionados trinta voluntários com indicação para cirurgia ortognática, os quais foram avaliados através de fotografias e exame físico, divididos aleatoriamente em três grupos de tratamento: grupo controle, que consistiu na utilização de medicação associada à aplicação de laser placebo nos pontos de acupuntura (Grupo 0) e os grupos experimentais, que consistiram na utilização de medicação associada à eletroacupuntura (Grupo 1) ou à laser-acupuntura (Grupo 2). Para cada tratamento experimental (eletroacupuntura ou laser-acupuntura) realizado em uma hemi-face do voluntário, foi feito o tratamento controle na outra hemi-face (n=15). O Grupo 0 consistiu no uso de medicações prescritas convencionalmente (decadron 8 mg; kefazol 1,0 g; zylium 50 mg; zofran 8 mg; dipirona 2 ml; keflex 500 mg; miosan 10 mg). No Grupo 1, além da medicação, foram colocadas agulhas descartáveis de acupuntura em pontos pré-determinados [E-4(dicang), M-CP-18(Jiachengjiang), VC-24(chengjiang), E-5 (daying), E-6 (jiache) e ponto A1(YNSA)] e os eletrodos foram conectados nas agulhas. Para a eletroestimulação, foram utilizados os seguintes parâmetros: TENS (estimulação nervosa elétrica transcutânea) que mensura modo acupuntura, tipo Burst, com variação de intensidade e frequência, com período (T) de 220 ms e frequência de 4 Hz, por 30 minutos, 2x/semana. No Grupo 2, além da medicação, a irradiação com laser de baixa potência (780 nm) foi realizada nos pontos de acupuntura, considerando os seguintes parâmetros:, área do spot de 0,04 cm2, 70 mW, 6 s/por ponto de irradiação, 0,42 J por ponto de irradiação, densidade de energia de 10 J/cm2, 2x/semana. Todos os voluntários foram avaliados antes de realizarem a cirurgia ortognática e foram tratados e avaliados a partir do sétimo dia pós-operatório. A avaliação foi realizada antes do início de cada sessão clínica de tratamento (antes da próxima intervenção terapêutica). O acompanhamento foi realizado duas vezes por semana, até completar quatro meses do início do atendimento. Foram realizados os seguintes testes: avaliação mecânica por pincelamento (pincéis nº 2 e nº 12); teste de discriminação de dois pontos com o compasso de duas pontas secas, para testar a discriminação de dois pontos (ambos testes táteis) e o teste elétrico pulpar, para estimular nervos intactos no complexo dentina-polpa através de uma aplicação de corrente elétrica na superfície do dente (teste para sensibilidade dolorosa). Todos os dados coletados nas sessões de avaliação foram transcritos para fichas específicas de avaliação e as respostas aos testes foram categorizadas em 2 níveis (com ou sem sensibilidade). Foram realizados os testes de Kaplan-Meier e as curvas de sobrevida foram traçadas para comparação entre os grupos. Os tempos medianos para o retorno da sensibilidade foram calculados e as diferenças entre as curvas de sobrevida dos diferentes tratamentos foi calculada através do teste de logrank. Também foram realizadas análises de regressão de Cox com fragilidade compartilhada para avaliar o efeito dos diferentes tratamentos no retorno da sensibilidade. O nível de significância adotado para todas as análises foi de 5%. Não houve diferenças estatisticamente significativas entre os grupos nos testes de discriminação de dois pontos, tátil do pincel nº 2, tátil do pincel nº 12 (na região de mucosa vestibular) e pulp tester de todas as regiões avaliadas. Porém, no teste tátil do pincel nº 12 houve diferenças estatisticamente significativas entre o grupo de eletroacupuntura e os demais grupos nas regiões de lábio inferior (p=0,024) e mento (p=0,028). Dentro dos limites do presente estudo clínico, pode-se concluir que apenas a eletroacupuntura foi capaz de influenciar positivamente, com uso do teste de pincelamento (nº12), o retorno da sensibilidade tátil nas regiões de mento e lábio inferior após cirurgia ortognática combinada e mentoplastia.

Influence of electroacupuncture and laser-acupuncture in treating paresthesia in patients submitted to combined orthognathic surgery and genioplasty

The objective of this randomized, blinded clinical trial was to observe the influence of electroacupuncture and laser-acupuncture in the return of tactile and pain sensitivity in patients submitted to combined orthognathic surgery and genioplasty. Thirty volunteers were selected with indication for orthognathic surgery, which were evaluated under photographs and physical examination and were randomly divided into three treatment groups: control group, which consisted of the use of medication associated with the application of laser placebo in acupuncture points (Group 0) and experimental groups, which consisted in the use of medication associated with electroacupuncture (Group 1) or laser-acupuncture (Group 2). For each experimental group (electro-acupuncture or laser-acupuncture) that was conducted on the volunteer's hemi- face, there was a control group on the other hemi-face (n = 15). Group 0 consisted on the use of conventionally prescribed medications (decadron 8 mg; Kefazol 1.0g, zylium 50 mg, 8 mg Zofran; dipyrone 2 ml; keflex 500mg, Miosan 10 mg). In group 1, medication was associated with acupuncture needles that were placed at predetermined points [E-4 (Dicang), F-HN-18 (Jiachengjiang), VC-24 (Chengjiang) E-5 (daying) E-6 (Jiache) and point A1 (YNSA)] and electrodes were connected to these needles. For electrostimulation, the following parameters were used: TENS (transcutaneous electrical nerve stimulation) which measures acupuncture mode, burst type, with intensity and frequency variation, with period (T) of 220 ms and a frequency of 4 Hz for 30 minutes, twice a week. In Group 2, in addition to the medication, irradiation with low power laser (780 nm) was done on acupuncture points by considering the following parameters: beam area of 0.04 cm2, 70 mW, exposure time of 6 s/point, energy of of 0.42 J per irradiation point, energy density of 10 J/cm2, twice a week. All volunteers were evaluated before the orthognathic surgery and were treated and evaluated after 7 days of the surgery. The evaluation was conducted before the beginning of each clinical session (before the next intervention). The monitoring of tactile and pain sensitivity was carried out twice a week up to four months from the beginning of the treatment. The following tests were performed: assessment by mechanical brushing (nº2 and nº12) and by two-points discrimination test using a bow compass (both tactile tests). Also, pulp electrical test was conducted to stimulate intact nerves of the dentin-pulp complex, by applying electric current on the tooth surface (pain test). Data collected throughout the clinical sessions were transcribed to specific evaluation forms and responses to tests were categorized into 2 levels (with or without sensitivity). Kaplan-Meier test was performed and survival curves were plotted for comparison between groups. The average time for the return of sensitivity was calculated for each group, and differences between the survival curves of the different treatments were calculated using the log rank test. Cox regression analyzes was also conducted with shared weakness to evaluate the effect of different treatments on the return of sensitivity. The significance level for all analyzes was set at 5%. There were no statistically significant differences between groups for the two-points discrimination test (brush nº 2 and nº 12) in the buccal mucosa region and for the pulp test on all evaluated regions. However, the tactile test using brush nº 12 revealed significant differences between the electroacupuncture group and the other groups when considering the lower lip (p = 0.024) and chin (p = 0.028) areas. Within the limits of the present clinical study, it can be concluded that only electroacupuncture was able to positively influence, using the brushing test (nº 12), the return of tactile sensitivity on chin and lower lip after combined orthognathic surgery and genioplasty.
