A escola assim como a família cumprem um papel importante no crescimento e desenvolvimento da criança, principalmente durante os primeiros anos de vida. A inclusão da faixa etária de 0 a 3 anos de idade no ciclo educacional no Brasil, ocorrida recentemente, e a ampliação do número de vagas nos centros educação infantil (CEI), tem resultado num maior tempo de permanência das crianças na escola. O objetivo deste trabalho foi avaliar a associação do tempo de permanência no CEI com a saúde bucal, hábitos de higiene bucal e práticas alimentares de pré-escolares do município de São Paulo. Para esse propósito, foi desenvolvido um estudo transversal com uma amostra por conveniência de 121 pré-escolares entre 3 a 4 anos de idade regularmente matriculadas em 04 centros educação infantil das Diretorias Regionais de Educação do Butantã e do Campo Limpo,vinculadas à Secretaria Municipal de Educação de São Paulo. Avaliou-se a saúde bucal das crianças através da prevalência, gravidade e fatores associados à cárie dentária; a maloclusão e a presença de placa visível ântero-superior, além de o consumo alimentar e do estado nutricional. Para as análisesestatísticas foram utilizados testes não paramétricos (teste chi quadrado e exato de Fisher para as variáveis categóricas e Mann Whitney para as variáveis numéricas). As associações entre a cárie dentária, presença de placa e maloclusão com as demais covariáveis foram analisadas através de regressão uni e multivariada de Poisson com variância robusta. O nível de significância adotado foi de 5%. Os resultados sobre a saúde bucal das crianças mostraram que a cárie dentária esteve associada com a visita da criança ao dentista(p=0,02);RP(IC95%)=2,40(1,14-5,06) com uma prevalência de 30,6% e um índice ceo-d de 0,94; entanto que a maloclusão esteve associado com o uso atual da chupeta(p=0,05);RP(IC95%)=1,85(1,00-3,43) tendo uma prevalência de 57,9% e sendo as alterações oclusais mais frequentes a sobressaliência e a mordida aberta anterior; e por último, a presença de placa esteve associado com a presença de cárie dentária modificada (p=0,03); RP(IC95%)=3,82(1,12-7,11);com uma prevalência de 40,5%. Mostrando-se em relação ao tempo de permanência da criança no CEI para cárie dentária, presença de placa e mal oclusão valores de p=0,82; 0,38; 0,61 respectivamente O contato de alimentos saudáveis e cariogênicos com a cárie dentária mostraram um valor de p=0,43;sendo para os alimentos saudáveis Média(DP)=5,15(1,44),e 5,22(1,30) com e sem presença de cárie dentária respectivamente;e para alimentos cariogenicos Média(DP)=5,15(1,44)e 5,22(1,30) com e sem presença de cárie dentária respectivamente.Em relação ao tempo de permanência da criança no CEI com o contato de alimentos mostraram valores de p=0,45(alimentos saudáveis) e p=0,61 (alimentos cariogênicos) sendo para alimentos saudáveis uma Média(DP)=5,35(1,39)para <=24 meses e 5,12(1,41) para >24 meses; e para alimentos cariogênicos Média(DP)=1,26(0,97) para <=24 meses e 1,19(0,97)para >24 meses.Entanto que o tempo de permanência da criança no CEI e o estado nutricional das crianças mostraram um valor de p=0,14 e valores maiores para o estado nutricional eutrófico (80,9%) em relação ao risco de sobrepeso (9,1%) e obesidade (10%).Conclui-se por tanto, na população estudada, que não foram encontradas associações significativas entre o tempo de permanência da criança no CEI e a saúde oral, sendo identificados uma maior prevalência de problemas de saúde bucal na região de Butantã, entanto que o contato com alimentos saudáveis das crianças, em casa,mostrou-se bem maior do que com alimentos cariogênicos, refletindo-se assim, no maior porcentagem das crianças com estado nutricional eutrófico.Salienta-se que CEI, desempenham um papel fundamental na sociedade, tentando equalizar o contexto social heterogêneo em que se vive, assim como promover a formação de hábitos saudáveis e proteção integral das crianças que se vê refletida na saúde bucal, principalmente durante os primeiros anos de vida.

Both the school and the family play an important role in the growth and development of the child, especially during the first years of life. The inclusion of the 0 to 3 age group in the recentlycompleted educational cycle in Brazil and the increase in enrollment in child education centers (CEC) has resulted in a longer period of time spent by children. The objective of this study was to evaluate the association of length time of staying in the public education system with oral health, oral hygiene habits and feeding practices of preschool children in the city of São Paulo. For this purpose,a cross-sectional study was developed with a convenience sample of 121 preschoolers between 3 and 4 years of age regularly enrolled in 04 child education centers in the Butanta and Campo Limpo regions, linked to the municipal education department from Sao Paulo. The oral health of children was evaluated through the prevalence, severity and factors associated with dental caries; the malocclusion and the presence of anterosuperior plaques, as well as food consumption and nutritional status. Statistical analyzes were performed using non-parametric tests (Chi square test and Fisher exact test for categorical variables and Mann Whitney test for numerical variables). The associations between dental caries, presence of plaque and malocclusion with the other covariables were analyzed by univariate and multivariate Poisson regression with robust variance. The level of significance adopted was 5%. The results on the oral health of children showed that dental caries was associated with the child's visit to the dentist (p=0.02);PR(CI95%)=2.40(1.14-5.06) with a prevalence of 30.6% and a ceo-d index of 0.94; However, malocclusion was associated with current pacifier use (p=0.05);PR(CI95%)=1.85(1.00-3.43), with a prevalence of 57.9% and changes more frequent occlusion of the overjet and anterior open bite; and finally, the presence of plaque was associated with the presence of modified dental caries (p=0.03); PR(CI95%)=3.82 (1.12-7.11); with a prevalence of 40.5%. It was shown in relation to the length time of staying of the child in the CEC for dental caries, presence of plaque and malocclusion values of p=0.82; 0.38 and 0.61 respectively The contact of healthy and cariogenic foods with dental caries showed a value of p=0.43; being for the healthy foods Mean(SD)=5.15(1.44), and 5.22 (1.30) with and without dental caries respectively, and for cariogenic foods Mean(SD)=5.15(1.44) and 5.22(1.30) with and without presence of dental caries, respectively. In relation to the length time of staying the child in the CEC with the food contactshowed values of p=0.45(healthy food) and p=0.61(cariogenic foods), were found for healthy foods. Mean(SD)=5.35(1.39) for <=24 months and 5.12(1.41) for>24 months; and for cariogenic foods Mean(SD)=1.26(0.97) for <=24 months and 1.19(0.97) for>24 months. However, the length time of staying the child in the CEC and the nutritional status of the children showed a value of p=0.14 and higher valuesfor the eutrophic nutritional status (80.9%) in relation to the risk of overweight (9.1%) and obesity (10%).It wasconcluded that, in the studied population, no significant associations were found between the child'slengthtime of staying in the CEC and oral health, but a higher prevalence of oral health problems was identified in the Butantã region. The contact with healthy foods of the children at home shown to be upper than the cariogenic foods, thus reflecting the higher percentage of children with nutritional status eutrophic. Emphasizing that CEC plays a fundamental role in society, but also trying to balance the heterogeneous social context in which they live, as well as promoting healthy habits and integral protection of children that is reflected in the oral health, especially during the first years of life.
