Os benefícios do aleitamento materno estão ligados às menores taxas de diarreia, infecções do trato respiratório, otite média e outras infecções, e menor mortalidade por essas doenças em crianças amamentadas. É importante que políticas públicas de saúde voltadas para a promoção, proteção e apoio ao aleitamento materno sejam efetivadas visando diminuir a incidência de doenças prevalentes na infância, dentre elas a diarreia. O presente estudo objetiva: identificar a prevalência dos tipos de aleitamento em crianças menores de 6 meses e de 6 a 12 meses de idade, em seguimento nas unidades com Estratégia Saúde da Família em Imperatriz-MA; identificar os casos de diarreia aguda notificados, associando os tipos aleitamento com os casos de diarreia aguda; associar as variáveis socioeconômicas e demográficas maternas, condições do pré-natal e parto com a prática da amamentação e a incidência de diarreia aguda; associar as variáveis referentes às características do recém-nascido com a prática de todos os tipos de amamentação e a incidência de diarreia aguda. A pesquisa foi descritiva e analítica, de natureza quantitativa e realizada de maneira transversal, entre junho de 2013 e julho de 2014. A amostra foi composta por 854 crianças menores de um ano e suas respectivas mães, cadastradas na Estratégia Saúde da Família. Para verificar associação entre os tipos de AM e o número de episódios de diarreia aguda, foi realizado o teste de qui quadrado e calculadas as razões de chance, considerando o intervalo de confiança de 95%. Os dados foram tabulados na planilha Excel 2013 e os testes realizados no programa SAS. A média das prevalências do AME em crianças de zero a seis meses foi de 32,0%, aos 12 meses apenas 15,0% das crianças estavam em algum tipo de aleitamento materno. A diarreia aguda esteve presente em 22,9% das crianças, e 11,2% necessitaram hospitalização, sendo que entre as crianças de 6 a 12 meses houve maior prevalência de diarreia aguda. As crianças menores de 6 meses que não mamaram tiveram 2,6 vezes mais chances de ter diarreia aguda e as crianças que estavam em aleitamento materno exclusivo tiveram menos diarreia aguda quando comparadas às crianças que estavam em outros tipos de aleitamento. A idade materna foi significante para a incidência de diarreia aguda. Crianças que usavam chupeta, mamadeira, água e chá tiveram influência negativa sobre a amamentação. Crianças que não usaram chupeta, mamadeira e água tiveram respectivamente 5, 16 e 8,5 vezes mais chance de serem amamentadas. Crianças que receberam mingau tiveram 2,7 vezes mais chances de desenvolver diarreia aguda. A introdução de alimentos saudáveis e não saudáveis antes dos 6 meses esteve presente entre as crianças estudadas. Quanto às crianças com mais de 6 meses, nem todas receberam alimentos saudáveis nessa faixa etária, contradizendo as recomendações de complementação alimentar nessa faixa etária. Assim, o presente estudo traz contribuições para a atenção à saúde de crianças e aponta que as ações de promoção, proteção e apoio ao aleitamento materno, recomendadas pelo Ministério da Saúde, ainda não foram implantadas plenamente no município

The benefits of breastfeeding are linked to lower rates of diarrhea, respiratory tract infections, otitis media and other infections, and lower mortality from these diseases in breastfed infants. It is important that public health policies aimed at promoting, protecting and supporting breastfeeding take their effect in order to decrease the incidence of childhood illnesses, among them diarrhea. This study aims at: identifying the prevalence of different types of breastfeeding in children under 6 months and 6-12 months of age, followed up on units applying the "Family Health Strategy" in Imperatriz-MA; identifying cases of acute diarrhea reported by associating types breastfeeding with cases of acute diarrhea; associating the socioeconomic and maternal demographic, prenatal and delivery conditions to the practice of breastfeeding and the incidence of acute diarrhea; associating the variables related to the newborn characteristics with the practice of all kinds of breastfeeding and the incidence of acute diarrhea. The research was descriptive and analytical, quantitative and performed in a transversal way, between June 2013 and July 2014. The sample consisted of 854 children under one year and their mothers who were enrolled in the "Family Health Strategy". To assess the association between the types of breastfeeding and the number of episodes of acute diarrhea, the chi-square test was performed and calculated the odds ratios, with a 95% confidence interval. Data were tabulated in Excel 2013 spreadsheet and testing in the SAS program. The average prevalence of exclusive breastfeeding in children up to six months was 32.0%. At 12 months only 15.0% of children were in some kind of breastfeeding. Acute diarrhea was present in 22.9% of children and 11.2% required hospitalization, and among children 6-12 months, there was a higher prevalence of acute diarrhea. Children under 6 months children not breastfed were 2.6 times more likely to have acute diarrhea and children who were exclusively breastfed had less acute diarrhea when compared to children who were in other types of feeding. Maternal age was significant for the incidence of acute diarrhea. Children who used a pacifier, bottle, water and tea were negatively related to breastfeeding. Children who did not use a pacifier, bottle and water were respectively 5, 16 and 8.5 times more likely to be breastfed. Children who received porridge were 2.7 times more likely to develop acute diarrhea. The introduction of healthy and unhealthy food before 6 months was present among the children studied. As for children over 6 months, not all received healthy foods at this age, contradicting complementary feeding recommendations to this age group. Thus, this study brings contributions to health care for children and points out that the promotion, protection and support of breastfeeding, recommended by the Ministry of Health, has not yet been implemented in the city
