A desinstitucionalização e as reformas políticas e de gestão atuais colocam a família como principal fonte de cuidado do portador de transtorno mental grave e de longa duração. A literatura indica que poucas pesquisas têm se dedicado a estudar o processo de ajustamento familiar durante o enfrentamento do transtorno mental. No que se refere à esquizofrenia, o primeiro episódio de adoecimento ocorre geralmente na adolescência. Trata-se de uma situação de estresse que desorganiza todo o grupo familiar, interrompe e pode modificar sua trajetória de vida. A partir daí, a família torna-se cuidadora e procura ajustar-se à nova situação de conviver com um ente familiar adoecido. O objetivo deste estudo foi descrever a construção desse cuidado através da trajetória das famílias que convivem com o diagnóstico de esquizofrenia há mais de dez anos. O referencial teórico-metodológico baseou-se na Teoria de Trajetória de Vida e orientou a escolha da história de vida como método de coleta e análise dos dados. Método: para contar a história da família participaram do estudo, como principais informantes, o portador de transtorno mental e até dois familiares cuidadores. Caso o portador do transtorno concordasse em participar da pesquisa, mas se recusasse a dar entrevista, ele deveria indicar pelo menos um familiar que pudesse contar a história por ele. Escolheu-se a entrevista como principal instrumento de coleta de dados. Foram entrevistadas 26 pessoas (18 familiares e 8 portadores de esquizofrenia), representando 14 famílias que convivem com o adoecimento há mais de dez anos. As entrevistas foram gravadas e realizadas pela pesquisadora no período de novembro/2010 a março/2011. O projeto foi aprovado pelo Comitê de Ética em Pesquisa do HCRP-FMRP-USP. Todos os participantes assinaram o Termo de Consentimento Livre e Esclarecido. A análise das entrevistas foi baseada na proposta do método da Teoria da Trajetória de Vida, que visa determinar através de uma linha do tempo os eventos importantes para o ajustamento familiar. Foram analisados três momentos: o passado, o presente e o futuro. Resultados: No momento passado foram descritos o sofrimento e as dificuldades do início do adoecimento e a aceitação como um evento importante para conseguir seguir em frente e se abrir para uma nova proposta. No presente, os relatos familiares descrevem momentos em que eles têm certeza de seu aprendizado. Novamente a aceitação é o elemento chave. Muitos familiares foram bastante afirmativos com relação a aceitar a doença como elemento chave para se relacionar com o adoecido e os outros membros da família. Alguns portadores de esquizofrenia confirmaram que quando a família aceita, ela é uma família melhor, em que ele se sente bem. Em relação ao futuro, há uma esperança nos avanços na psicofarmacologia e em encontrar na família os recursos para o cuidado. Conclui-se que esta pesquisa atendeu aos objetivos propostos e aponta para a urgente necessidade de se ouvir o portador de transtorno mental e seus familiares sobre esta "nova" responsabilidade da família: cuidar de seu ente querido.

The deinstitutionalization and the currently political and management reforms put the family as the main source of care for people with severe and long-lasting mental illness. The literature indicates that little research has been done to study the process of family adjustment coping with mental disorder. With regard to schizophrenia, the first episode of illness usually occurs in adolescence. It is a stressful situation which disrupts the whole family group, interrupts and can change its life trajectory. From there, family members become caregivers and try to adjust to the new situation of living with a family member being ill. This study aimed to describe the construction of this care through the trajectory of families living with people suffering schizophrenia for over ten years. The theoretical and methodological framework was based on the Trajectory Theory and guided the choice of life history as a method of collecting and analyzing data. Method: the mental disorder patient and up to two family caregivers participated in the study as key informants to tell the story of the family. If the carrier of the disorder agreed to participate in the research, but refused to be interviewed, he/she should appoint at least one family member who could tell the story for him/her. The interview was chosen as the main instrument for data collection. 26 people (18 families and 8 people with schizophrenia) were interviewed, representing 14 families living with people suffering schizophrenia for over ten years. The interviews were recorded and performed by the researcher in the period from November/2010 to March/2011. The project was approved by the Research Ethics Committee of the Hospital das Clínicas of the University of São Paulo at Ribeirão Preto Medical School. All participants signed the consent form. Analysis of the interviews was based on the method proposed in the Trajectory Theory, which seeks to determine through a timeline of important events for the family adjustment. Three times were analyzed: past, present and future. Results: regarding the past, it was described the suffering and difficulties of first time and acceptance as an important event to move on and open to a new proposal. At present, the family reports describe times when they are sure of their learning. Again, the acceptance is the key element. Many family members were very assertive about accepting the disease as a key element to have a relationship with the ill person and other family members. Some people with schizophrenia have confirmed that when the family accepts, it is a better family which he/she feels good. In relation to the future, there is hope in the advances in psychopharmacology and find the resources to care in the family. It is concluded that this study met the proposed objectives and showed the urgent need to listen to the mentally ills and their families about this "new" family responsibility: taking care of their loved one.
