Introdução: A esclerose múltipla (EM) é uma doença autoimune que afeta o sistema nervoso central, de forma predominante, a substância branca, resultando em inflamação e desmielinização. Alguns estudos demonstraram que o transplante de células tronco hematopoiéticas (TCTH) é capaz de suprimir a atividade inflamatória em pacientes com doenças autoimunes (DAI). A incapacidade funcional causada pela EM é um dos fatores mais relevantes que impactam negativamente a vida do paciente. Objetivos: Análise crítica da evolução da capacidade funcional, por meio do EDSS, dos pacientes com esclerose múltipla submetidos ao transplante de medula óssea; Avaliar a evolução da capacidade funcional dos pacientes tratados com TCTH por meio da escala expandida do estado de incapacidade (EDSS) nos períodos pré-TMO e follow-up (após 6 meses, e a cada 1 ano). Verificar a presença de gadolínio pré e pós -transplante. Métodos: trata-se de um estudo longitudinal retrospectivo onde foram coletados dados de 82 pacientes com EM submetidos ao TCTH com idade média de 38 anos e acompanhados durante o tempo médio de 11,3 anos. Utilizamos dados do EDSS pré, 6 meses e anual pós -transplante, se realce ao gadolínio, presença de surto e progressão da doença, e do teste de caminhada de 6 minutos (TC6) pré e pós-transplante. Os pacientes foram divididos em três grupos, de acordo com a forma clínica apresentada no transplante: remitente-recorrente (RR), secundariamente progressiva (SP) e primariamente progressiva (PP). Resultados: Dos pacientes transplantados, 67% SP, 27%RR e 6%PP. Esses pacientes revelaram EDSS médio igual ou menor que 6,0 pontos. A progressão da doença se deu em 40% (2)PP, 33,33%(7) RR e 44,44%(24) SP no período maior ou igual a 6 meses. Na relação EDSS x tempo, os pacientes RR com realce ao gadolínio sustentaram melhor a resposta ao transplante, já que conseguiram sustentar o escore do EDSS inalterado por mais tempo que as demais formas clínicas. Os pacientes SP apresentam pouca variação do EDSS ao longo do tempo no período pós-transplante. O impacto da incapacidade da marcha foi maior nos pacientes PP, todos apresentavam alguma restrição ao deambular. Conclusão: Os pacientes RR com realce ao gadolínio submetidos ao TCTH sustentam a resposta ao procedimento por mais tempo e com melhor evolução neurológica. Os pacientes SP apresentam pouca variação na pontuação do EDSS no póstransplante. Outros instrumentos poderiam ser utilizados em associação com o EDSS para identificar discretas alterações na incapacidade funcional dos pacientes com EM submetidos ao TCTH

Introduction: The multiple sclerosis (MS) is an autoimmune disease that affects the central nervous system, damaging mainly the white substance, resulting in inflammation and demyelination. Some researches showed that the hematopoietic stem cell transplantation (HSCT) is capable of suppressing the inflammatory activity in patients who have autoimmune disease. The functional inability caused by MS is one of the most relevant reasons that adversely affects the life of the patient. Aims: critical analysis of disability status score, through the DSS, in patients who have multiple sclerosis submitted to bone marrow transplant. Evaluate the evolution of the functional capacity of the patients who were under HSCT treatment through DSS during the pre-bone marrow transplantation (BMT) and follow-up term (after 6 months, and every year). Verify the presence of gadolinium pre and post-transplant. Methods: it is a retrospective longitudinal study which was collected some data from 82 patients with MS submitted to HSCT which average age is 38 years old and have been watched for about 11,3 years. We used DSS data, pre, 6 months and annual and post-transplant, if there is enhancement to gadolinium, outbreak presence and the disease progress, the six-minute walk test pre and post-transplant. The patients were split in three groups, according to their clinical form shown in the transplant: sender-applicant, secondarily progressive and primarily progressive. Findings: When it comes to the transplanted patients, 67% secondary progressive, 27% relapsing-remitting and 6% primary progressive. These patients showed DSS equal or lower than 6.0 points. The disease progress occurred in 40% (2) primary progressive, 33,33%(7) relapsing-remitting e 44,44% (24) secondary progressive during the period of 6 months or more. When it comes to DSS x Time, the senderapplicant patients with enhancement to gadolinium had a better outcome to the transplant, since they could maintain the DSS score unchanged for a longer period of time compared to other clinical forms. The patients secondarily progressive show little DSS variation as the time goes by during the post-transplant term. The inability walk impact was higher in primarily progressive patients, all of them had some wander restriction. Conclusion: The relapsing-remitting patients with enhancement to gadolinium submitted to HSCT have a longer response to the procedure and more neurological evolution. The secondary progressive patients show little variation when it comes to DSS score in a post-transplant scenario. Other instruments could have been used with DSS in order to identify discrete changes in functional disability in patients with MS submitted to HSCT
