Diabetes mellitus (DM) é uma das doenças crônicas não-transmissíveis mais comuns em praticamente todos os países. As principais complicações agudas do DM são: cetoacidose diabética (CAD), síndrome hiperosmolar hiperglicêmica (SHH) e hipoglicemia, causadas pelo controle ineficaz da taxa glicêmica. Apesar de serem evitáveis, ocorrem com frequência, levando os pacientes à internação e alguns a óbito. Desta forma, o objetivo principal deste estudo foi analisar a tendência das internações, pelo sistema público de saúde, decorrentes de complicações agudas do DM. Isso foi realizado em cinco segmentos diferentes: no Brasil, na região Sudeste, no estado de São Paulo, no Departamento Regional de Saúde XIII e no município de Ribeirão Preto, no período de 2002 a 2016, utilizando como fonte de dados as autorizações de internação hospitalar (AIH) provenientes do Sistema de Informações Hospitalares do SUS (SIHSUS), disponíveis livremente no Departamento de Informática do SUS (DATASUS). Os dados foram analisados segundo algumas variáveis como sexo, faixa etária, média de permanência hospitalar e letalidade. Ao longo destes quinze anos, observou-se diminuição na proporção de internações por complicações agudas do DM em relação ao total de hospitalizações, no Brasil e na região Sudeste. Estado de São Paulo, DRS XIII e Ribeirão Preto, mostraram aumento desta proporção no último triênio analisado (2014-2016), após períodos de queda. Verificou-se aumento da letalidade hospitalar em todas as regiões, exceto Ribeirão Preto. Em relação ao sexo, as mulheres apresentaram maiores proporções de internação do que os homens nas cinco regiões analisadas, ao longo dos quinze anos. Notadamente, houve incremento do número de internações na faixa etária mais jovem (0-19 anos), assim como no número de óbitos nessa faixa etária. Não houve diferença quanto à média de permanência hospitalar em relação às internações com desfecho óbito e desfecho alta, ao longo dos quinze anos de estudo e entre as cinco regiões. A diminuição global do número de internações por complicações agudas do DM sugere melhoria do atendimento básico, entretanto o aumento da letalidade hospitalar e a elevação da incidência no grupo etário dos mais jovens são motivos de preocupação e indicam um longo caminho a ser percorrido no que diz respeito ao manejo adequado do Diabetes mellitus.

Diabetes mellitus (DM) is one of the most common chronic noncommunicable diseases in virtually every country. The main acute complications of DM are: diabetic ketoacidosis, hyperglycemic hyperosmolar syndrome and hypoglycemia, caused by ineffective glycemic control. Although avoidable, they often occur, leading patients to hospitalization and some to death. The aim of this study was to analyze the trend of hospitalizations due to acute complications of DM. That was done in five distinct sections: Brazil, the Southeast region, the state of São Paulo, the XIII Regional Health Department (shortened to DRS XIII in Portuguese) and the city of Ribeirão Preto, in 2002 to 2016 period, using data from the SUS (Sistema Único de Saúde or Unified Health System) Hospital Information System (SIHSUS) freely available at the Department of Informatics of SUS (DATASUS) as data source. The data were analyzed according to some variables, such as sex, age group, length of hospital stay and lethality. During these fifteen years, there was a decrease in the proportion of hospitalizations due to acute complications of DM in Brazil and in the Southeast region. São Paulo state, DRS XIII and Ribeirão Preto, showed an increase in these proportions in the last three years analyzed (2014- 2016), after a long period of decline. There was an increase in hospital lethality in all regions except Ribeirão Preto. Regarding gender, women presented higher admission rates than men in the five regions analyzed over the period. Notably, there was an increase in the number of hospitalizations in the younger age group (0-19 years), as well as in the number of deaths in this age group. There was no difference in the mean of the length of hospital stay in relation to hospitalization outcome (medical release or death) over the fifteen years of study and among the five regions. The global decrease in the number of hospitalizations due to acute complications of DM suggests an improvement in basic care. However, the increase in hospital mortality and the increase in the incidence in the younger age group are reasons for concern and indicate a long way to go regarding the correct management of Diabetes mellitus.
