O Câncer de Próstata (CaP) é a neoplasia maligna mais prevalente no sexo masculino, o segundo tipo de câncer mais diagnosticado em homens e o quarto mais comum no mundo. No Brasil, o CaP é o tumor maligno mais diagnosticado e o segundo que mais leva ao óbito. Deste modo, o teste e descoberta de biomarcadores moleculares que possam melhorar o diagnóstico, seguimento e prognóstico da doença, além de auxiliar no desenvolvimento de novas modalidades de tratamento tem se tornado um dos objetivos essenciais das pesquisas neste campo. O presente estudo determinou a expressão relativa dos genes PCA3, GOLPH2, SPINK1, ERG e TDRD1 em amostras de urina pós massagem prostática de pacientes encaminhados ao Ambulatório de Urologia da Universidade de São Paulo (HC-FMRP-USP) com indicação de biopsia prostática para investigação de CaP. Os pacientes que participaram no estudo foram divididos em dois grupos: pacientes com CaP do tipo adenocarcinoma (n=46) e pacientes com HBP (n=80), estes últimos constituíram o grupo controle para testar a especificidade dos marcadores. A análises estatísticas demonstraram que existe uma diferença significativa na expressão do gene GOLPH2 e SPINK1 entre os pacientes com CaP e HPB (p=0,003; p=0,035). A área obtida sob a curva ROC para o teste com o gene GOLPH2 foi 0,692, a sensibilidade 54,35% e a especificidade 77,14%. O teste com gene SPINK1 apresentou uma área sob a curva ROC de 0,637, e a sensibilidade e a especificidade foram de 72,34% e 57,14% respectivamente. Conclui-se que o teste quantitativo de expressão relativa dos genes GOLPH2 e SPINK1 aponta ser um biomarcador útil na diferenciação entre pacientes com CaP ou HPB. Sugere-se a criação de um painel com estes dois marcadores com o objetivo de criar um teste que seja altamente sensível e específico.

Prostate cancer (CaP) is the most prevalent malignant neoplasm in men, the second most diagnosed type of cancer in men and the fourth most common in the world. In Brazil, CaP is the most diagnosed malignant tumor, and the second most frequent malignant tumor. In this way, testing and discovering molecular biomarkers that can improve the diagnosis and prognosis of the disease, besides helping to develop new treatment modalities, has become one of the essential objectives of the research in this field. The present study determined the relative expression of PCA3, GOLPH2, SPINK1, ERG and TDRD1 genes in urine samples after prostate massage of patients referred to the Urology Clinic of the University of São Paulo (HCFMRP-USP) with indication of prostate biopsy for prostate cancer investigation. Patients who participated in the study were divided into two groups: patients with prostate cancer (n = 46) and patients with benign prostatic hyperplasia (n = 80), the last one, was the control group to test the specificity of the markers. Statistical analyzes demonstrated that there is a significant difference in the expression of the GOLPH2 and SPINK1 gene among patients with prostate cancer and benign prostatic hyperplasia (p = 0.003; p = 0.035). The area under the ROC curve for the GOLPH2 gene test was 0.692, the sensitivity was 54.35%, and the specificity was 77.14%. The SPINK1 gene test showed an area under the ROC curve of 0.637, and the sensitivity and specificity were 72.34% and 57.14%. It is concluded that the quantitative test of relative expression of the genes GOLPH2 and SPINK1 points out to be a useful biomarker in the differentiation between patients with prostate câncer and benign prostatic. It is suggested to create a panel with these two markers in order to create a test that is highly sensitive and specific.
