Os testes para diagnóstico da raiva devem apresentar rapidez, alta sensibilidade e especificidade na obtenção dos resultados. No entanto, em amostras de equinos, muitas vezes, pode-se observar uma sensibilidade na imunofluorescência direta (IFD) mais baixa quando comparada com aquela de outras espécies. Em decorrência disto, a implantação de novas ferramentas no diagnóstico poderá diminuir o número de resultados falso negativos. No presente estudo foi utilizado o SNC de 80 equinos originários de municípios do estado de São Paulo, encaminhados para a Seção de Diagnóstico da Raiva do Instituto Pasteur no período de abril de 2017 a abril de 2018. As amostras utilizadas foram processadas para IFD, isolamento viral em cultivo de células (IVCC), transcrição reversa seguida da reação em cadeia da polimerase (RT-PCR) e transcrição reversa seguida da reação em cadeia da polimerase em tempo real (RT-qPCR). Das 80 amostras analisadas de SNC de equinos, 25 (31,25%) apresentaram resultado positivo para raiva em pelo menos uma das técnicas mencionadas. Ao avaliar a positividade, comparando os resultados entre as técnicas, a IFD apresentou 96% (24/25), IVCC 44%(11/25), RT-PCR 100% (25/25) e RT-qPCR 100% (25/25). Analisando 161 estruturas do SNC separadamente [córtex, hipocampo, cerebelo, tronco cerebral (mesencéfalo, ponte e bulbo) e medula cervical, espinhal e lombar] de 25 cavalos positivos, as taxas de positividade foram para 72,0%, 89,4% e 75,2% para IFD, RT-PCR e RT-qPCR, respectivamente. Cerca de 33% das estruturas do SNC (53/161) apresentaram rara presença de antígeno, com apenas uma a três pequenas inclusões em todo o campo microscópico pesquisado, sendo classificadas como fracas positivas. O mesencéfalo apresentou a melhor distribuição viral nas estruturas do SNC tanto pela técnica de IFD quanto pelas técnicas moleculares (RT-PCR e RT-qPCR). Esse fato sugere que a variabilidade na intensidade da distribuição do vírus em diferentes partes do SNC (padrões neuroinvasivos) em equinos pode influenciar a detecção de RABV. A detecção de presença viral em glândulas salivares mostrou-se positiva, indicando o potencial destes animais atuarem como fonte de infecção para raiva.

Tests to diagnose rabies must be rapid, highly sensitive and highly specific in obtaining the results. However, a lower sensitivity to the direct fluorescent antibody (DFA) may be observed in equine central nervous system (CNS) samples when compared to those of other species. The use of new tools for rabies diagnosis in equine specimens may reduce the occurrence of false-negative results. In the present study, the CNS from 80 horses were obtained from April 2017 to April 2018 as a result of rabies surveillance operations established in the São Paulo state. The samples were tested at the Pasteur Institute of São Paulo's Rabies Diagnosis Section by DFA, rabies tissue culture infection test (RTCIT), RT-PCR, and RT-qPCR. Out of the 80 analyzed CNS samples from horses, 25 were positive for rabies (positivity rate = 31.25%) at least in one of the mentioned techniques. Of these positive cases, 96% (24/25) were confirmed by using the DFA test. 25 CNS samples were positive for both RT-PCR and RT-qPCR, with 100% sensitivity. Only 11 of the 25 CNS samples were positive (44%) by RTCIT. When the 161 CNS structures were analyzed separately [cortex, hippocampus, cerebellum, brainstem (midbrain, pons, and bulb) and cervical, lumbar and spinal cord] from 25 positive horses, the positivity rates were 72.0%, 89.4% and 89.6% for DFA, RT-PCR, and RT-qPCR, respectively. About 33% CNS structures (53/161) showed a rare presence of antigen, with only one to three small inclusions in the entire microscopic field searched, being classified as weak positive. Midbrain showed the best viral distribution in the CNS structures by both the DFA and the molecular techniques RT-PCR /RT-qPCR. This fact suggests that the variability in the intensity of virus distribution in different parts of the CNS (neuroinvasive patterns) in horses may influence the detection of RABV. The detection of RABV in salivary glands was positive, indicating the potential of these animals as a source of infection for rabies.
